PLAN MANAGEMENT

Here at Disability Chioce Matter participation and inclusion is a central part of our mission statement. It is essential that the participant plays a major part in all decisions regarding their support and care.

Every one of our participants has a management plan setting out requirements and goals for both the short and long term future. We work closely with all our participants to set realistic, achievable targets. We regularly review and if necessary revise our goals in consultation with our participant.
