SUPPORT COORDINATION
Support coordination is a capacity building support which helps you to understand and use an NDIS plan and to pursue your goals. It connects you with NDIS providers, community, mainstream and other government services. It also helps you to build your confidence and coordinate your support.
Our core mission and values are all client focused. The care and support offered to all our clients is our number one focus. Our primary aim is to give the choice and control to the participant and we feel the way to achieve this is by working together to get the best results possible.
There are 3 levels of support coordination:-
1.Support Connection – This aims to build your ability to connect with informal, community and funded support enabling you to maximise your plan and pursue your goals.
